It’s often easy for GPs to have little information on the admission, current status and discharge of one of their patients to acute hospital or community based services; information flows slowly, if at all, and the opportunity for support and co-ordinated care can be delayed or lost.

With Cayder PFM, a GP can be provided with authorised access to a complete list of their patients currently undergoing treatment at a hospital or community care provider. And, in conjunction with the provider, data views can be prepared that allow GPs to access key status information on the current progress of their patient, length of stay, expected discharge date and on-going care requirements. Any treatment and/or care delays can also be easily highlighted enabling facilitated support to be provided as and when necessary.
